Who Will Be Our Last Guest of the Year?

As the year comes to an end, I often wonder who the final guest of the year will be.
Last year’s final guest is unforgettable.
On December 31st, I received a phone call around 10 a.m. It was regarding a Canadian firefighter who was traveling in Japan with his extended family. I was already treating a Japanese client at the time. The call came through a nationally licensed tour guide who was coordinating the family’s trip.
Because it was a large family, communication was complicated. The guide repeatedly called both the guest and me, explaining that the situation was urgent. I offered an appointment at 11:00 a.m., but they said it was too soon and asked for a later time, mentioning they might arrive slightly after 11:30.
I waited.
No one arrived at 12.
No one arrived at 1.
Eventually, after calling the guide again, the guest finally arrived at 2:30 p.m.—four hours later.
Because I run my practice alone, I had carefully finished my previous appointment early to be ready. Waiting that long on New Year’s Eve meant I couldn’t do anything else that day.
The guest had said he was in severe pain, yet arrived casually holding a takeaway Starbucks drink. When asked about his condition, he said it wasn’t that bad anymore.
In that moment, I understood.
He wasn’t truly seeking urgent treatment.
He simply wanted a short break from a large family trip—some quiet time with his wife, maybe a coffee, maybe a massage, a small “date” in a foreign country.
As someone who has experienced traveling with large families, I understood the feeling.
However, my clinic is a one-person practice. I cannot accept vague arrivals or open-ended waiting times. His wife, who was genuinely worried, carefully explained his symptoms, likely trying her best to manage the family schedule.
I was glad my clinic could serve as a small sanctuary—but waiting four hours on New Year’s Eve was too much.
That experience taught me an important lesson.
From now on, during busy seasons, appointments will require a clearly specified arrival time. If guests cannot arrive by that time, the clinic will close for the day.
